The car was sitting on a jack with the rear tire removed, and kids were running around nearby, which made it an immediate safety concern.

So the instruction was clear:

“James, you need to put the tire on the car. The grand babies run around and I don’t want them to bump into it or crawl under it and get hurt.”

The expectation was straightforward. The tire gets installed on the car, secured properly, and the situation is no longer a risk.

James acted right away. He didn’t ask any questions or restate what he was going to do. Instead, he took the tire and placed it underneath the car, laying it flat so that if the jack failed, the car would fall onto the tire.

From his perspective, he made the situation safer. From the other side, the actual problem wasn’t solved. The tire still wasn’t on the car, and the car was still sitting on the jack.

The situation changed, but the outcome didn’t.

What actually broke

This was a Validation failure.

The instruction was given and action followed immediately, but interpretation was never surfaced. Both people were operating from different definitions of what it meant to “fix the problem.”

One definition was to fully install the tire on the car. The other was to reduce the risk if something went wrong.

Because no one checked that interpretation before action, the gap stayed hidden until the work was already done. What looks like partial effort is actually a difference in understanding.

What to fix

The fix is to apply Validation before execution by forcing interpretation into the open.

“Walk me through what you’re going to do to fix this.”

Where this shows up

This shows up anywhere instructions are given and acted on quickly. It happens in delegation at work, in leadership conversations, and in everyday situations at home.

Something gets done, but not the right thing. The output doesn’t match the expectation, even though effort was there.

Why it keeps happening

Because agreement is treated as understanding. When someone doesn’t push back or ask questions, it feels like alignment, but nothing has actually been confirmed.

Without Validation, people default to their own interpretation and move forward with it. The system doesn’t break at execution—it breaks before execution when interpretation is assumed instead of verified.

What happened next

Nothing changed. The tire remained on the ground, the car stayed on the jack, and the original safety issue was still present.

What we’re seeing

This is a repeatable pattern:

Skipped Validation → Partial compliance → Misalignment

People don’t ignore instructions. They reinterpret them and then execute based on that interpretation.

Trigger Signal

When someone moves straight into action without explaining their understanding, or when there’s a pattern of “doing something, but not fully,” that’s the moment to intervene.

Ask for their plan before they begin.

Preventive Practice

After giving direction, require a brief restatement of what will be done, how it will be done, and what “done” looks like. This forces alignment before execution instead of after.
